Sandra Doyle, Advice on Prescription Plus Coordinator at Citizens Advice Liverpool has recently joined the HOP network. Here she explains the programme for HOP members who would like to know how to connect in:
“Advice on Prescription: Ways to Wellbeing receives referrals from GP teams and trusted partners in Liverpool. When we receive the referral, our link workers contact the patient to discuss and agree an individualised ‘wellbeing plan’. The link worker provides ‘ways to wellbeing’ that focus on reducing loneliness, increasing physical activity and providing meaningful access to creative outlets and supporting self-confidence and economic resilience.
“It would be great to know about the activities and services that HOP members provide. The Livewell Directory is the first place our link workers look for activities for our clients. We may not have information about members not listed on the directory.
“HOP members can contact Healthwatch Liverpool to list their activities and services on the directory. Alternatively, members can send information about their services and activities to the following e-mail address: firstname.lastname@example.org.
“We are already referring our clients to some HOP members such as Liverpool Cares and Age Concern.”
